Carbon Footprint Supplier Assessment Questionnaire

This document is a basic questionnaire to assess and report on the environmental and social impacts of pharmaceutical suppliers' practices. The questionnaire provides a general survey of impacts and practices in terms of carbon emissions, packaging, water usage, waste , handling & management (discharges of chemicals and active pharmaceutical ingredients), traceability, human rights and work conditions and other certifications.

This questionnaire can serve as a practical tool for health care organisations and their suppliers, especially small and medium enterprises in the health products area, to provide basic qualification and training to suppliers on some sustainable and low carbon practices. It complements the Sectoral Guidance document for suppliers of self-care products.

Intended users

• Suppliers of self-care and health care products, including companies involved in the manufacturing, packaging, distribution and retail of medicines and other products.
• Administrators of health care organizations and entreprises working with suppliers, including product distributors and manufacturers.
